Steven Tabler (64) of Wapakoneta was ice fishing when he slipped and fell through the ice, becoming trapped in the water.

When first responders arrived at the scene, a hole had been cut in the ice and fishing gear was located nearby. Tabler was pronounced dead at the scene.

The incident remains under investigation.
