This nationally recognized award honors health care professionals who have made a significant impact on the lives of under served populations through meaningful health care and social services interventions, while advancing health equity for all patients.
Heather’s work across Auglaize and Mercer Counties reflects the very heart of Grand Lake Health System’s mission. Through her leadership, compassion, and collaboration, she has helped improve access to preventive care, address social barriers to health, and build lasting community partnerships that strengthen the well-being of the people we serve.
The RISE Healthy Communities Summit is a national gathering of health care leaders, community organizations, policymakers, and innovators dedicated to whole-person, community-centered solutions that improve health outcomes nationwide.
